The completed form should be faxed to the hotel of your choice. Inquires to the hotels can be made by email, but completed booking forms should not be sent by email. Please write or type information in full, one form for each participant.

Credit card guarantee
Payment generally by credit card in USD. Payment by bank transfer may be accepted in exceptional circumstances, and upon request by contacting FIDIC before arranging payment.
Credit cards will be debited for a one-night stay as a deposit to secure a booking. I authorise one-night to be deducted from my credit card to guarantee the reservation. For reservations before 1 July 2005, this will be one-night at the special rate. After this date, a higher rate may apply subject to availability.

Payment by bank transfer
Payment by bank transfer may be accepted in exceptional circumstances, and upon request by contacting FIDIC before arranging payment. The full amount must be paid, plus USD 20 for bank charges. Extraordinary bank charges arising from special handling will be invoiced separately. For reservations before 1 July 2005, this will be the total amount based on the special rates. After this date, FIDIC will inform participants upon request of the total amount to be paid.

Terms and Conditions

Rooms will be allocated on a first come, first served basis.
Reservations will be confirmed by the hotel; based on availability.
Reservations, credit card guarantees and payments by bank transfer will be received after 1 July 2005 will be handled, but the desired accommodation and/or special rates cannot be guaranteed.
Check-in time is 13:00; check-out time is 12:00 noon. Early check-in and late check-out upon request, based on availability.
Reservations can be cancelled without 72 hours prior to arrival date, otherwise one (1) room night will be charged.
For a no show, one (1) room night will be charged and any remaining night cancelled.
In the case of payment by bank transfer, any refunds will incur an administration fee of USD 30 to cover transfer and handling charges.
