This reservation form [ PDF version ] is only valid for participants at the FIDIC-EFCA-AHCEA 2006 Conference, Budapest (24-27 September 2006). The completed form should be faxed or emailed to the hotel of your choice before 15 August 2006 to benefit from the conference rates given below. The full amount owing for the booking will be debited on 15 August 2006 from the credit card used to guarantee the booking, and will not be refunded in the case of a cancellation or no-show. As the number of rooms available at the conference rates is limited, please book as early as possible. Subject also to availability, the rates will apply for bookings that start and end between 21 September 2006 and 30 September 2006. The organizers do not guarantee that the rates given below are the most advantageous available. Please write or type information in full, one form for each conference registration.

Credit card guarantee
Payment generally by credit card in Euro. Payment by bank transfer via FIDIC may be accepted in exceptional circumstances, and upon request by contacting FIDIC before arranging payment (see below).
Credit cards will be debited for a one-night stay as a deposit to secure a booking in the case of cancellation after 15 August 2006. The rate used will be the rate confirmed on making the booking. Before 15 August 2006 and subject to availability, this will be the conference rate given above. The full amount will be debited on 15 August 2006, and will not be refunded in the case of cancellation or a a no-show. Settlement is normally by cash or credit card unless arrangements have been made with FIDIC for settlement by bank transfer.
I authorise one-night to be deducted from my credit card to guarantee the reservation. Subject to availability, for reservations before 15 August 2006, this will be for one night at the conference rate, otherwise at the rate confirmed for the booking.

Payment by bank transfer
Payment by bank transfer may be accepted by the organizers in exceptional circumstances, and upon request by contacting FIDIC before arranging payment. The full amount must be paid, plus Euro 20 for bank charges. Subject to availability, before 15 August 2006 this will be the total amount based on the conference rates given above. FIDIC will inform participants when they make a request for payment by bank transfer of the total amount to be paid. Extraordinary bank charges arising from special handling will be invoiced separately. FIDIC will guarantee the booking by credit card. The full amount paid will not refunded in the case of cancellation 15 August 2006 or a no-show. The rate used will be the rate confirmed on making the booking.

Terms and Conditions

Please refer to the Terms and Conditions for the selected hotel. In general, rooms will be allocated on a first come, first served basis.
Reservations will be confirmed by the hotel; based on availability.
Reservations and credit card guarantees will be handled by the hotels, but the desired accommodation and/or the conference rates cannot be guaranteed.
Check-in time is 13:00; check-out time is 12:00 noon; early check-in and late check-out upon request, based on availability.
Reservations can be cancelled without charge up to 15 August 2006. After this date the full amount of the booking will be debited from the credit card used to guarantee the booking at the rate that was confirmed for the booking. For bookings made after 15 August 2006 and then cancelled the full amount of the booking will be debited from the credit card used to guarantee the booking at the rate that was confirmed for the booking.
For a no-show, the entire amount of the booking at the rate that was confirmed for the booking will be debited from the credit card used to guarantee the booking.
In the case of payment by bank transfer via FIDIC, refunds or changes will incur an an administration fee of Euro 20 to cover transfer and handling charges.
